The Transitional Care Program (TCP)
aims to help older people to improve their independence and
confidence after a stay in hospital.
Transitional Care Programs provide
a package of care services including low intensity therapy and
personal and/or nursing care as part of an ongoing but slower
recovery process outside the hospital environment.
This means that following a
hospital stay you and your family or carer have time to consider
your long-term care arrangements, which may include returning home
with community support or entering an aged care facility that
caters to the level of care you are assessed as requiring.
Transitional care is usually
provided for a period of up to 12 weeks, with a possibility to
extend to 18 weeks if you are assessed as needing an extra period
of therapeutic care. The average period of care is usually about
eight weeks.
